Grants to Iowa hospitals, health centers, community-based organizations, and integrated delivery systems to improve health care in rural areas. The purpose of this program is to increase access to comprehensive, quality health care. Successful applicants will create Centers of Excellence that encourage innovation and collaboration among regional health care providers in rural areas. The applicant's proposed service delivery region must include four or more contiguous rural counties or rural portions of counties.
Centers of Excellence are innovative care models that specialize in specific areas of medicine, offer a variety of services and treatments, and create regional access to specialty care with a focus on creating access to specialty care in rural areas where access is currently lacking.
The Department is actively working to strengthen its capacity to address health inequities in Iowa. Health equity is defined as supporting opportunities for everyone to live the healthiest life possible through the reduction of barriers in environmental, social, economic and other conditions which impact health. This program promotes health equity through working to reduce disparities and improve the health and overall well-being of rural communities by increasing access to specialty care in rural areas.
Priority will be given to applications that propose to service a large number of rural residents within their service region and must demonstrate accessibility of services within their proposed region.
The goal of this program is to demonstrate regional collaboration to assess targeted medical needs of local residents, and establish partnerships to leverage resources and develop a business model for long-term sustainability of Centers of Excellence providing access to specialty care for rural communities.
